Standard Form (SF) 3107-Schedule D is entitled “Agency Checklist of Immediate Retirement Procedures.” This form must be completed by your Agency on your behalf and included with your application for FERS disability retirement. All federal employees who are seeking retirement benefits regardless of the type (e.g., disability, MRA + 10, discontinued service, etc.) must submit a completed SF 3107-Schedule D with their application package. Once your Agency completes this form, they should return the original (and all supporting documentation) to you for your review. Please take the time to review this form carefully for accuracy and completeness. For example, you want to make sure that your last day in a pay status (as listed on SF 3100 or SF 3100A) is correct so that you won’t be denied any retroactive payments to which you are entitled.
