Standard Form (SF) 3107 is entitled “Application for Immediate Retirement: Federal Employees Retirement System.” This form must be completed by you, the employee, and included with your application for FERS disability retirement. All federal employees who are seeking retirement benefits regardless of the type (e.g., disability, MRA + 10, discontinued service, etc.) must complete SF 3107. Note that Section I contains a checklist of documents to include with SF 3107. For example, if you have previous military service, you are required to attach a copy of your discharge certificate. And please don’t forget to sign and date Section I, or you may experience delays in the processing of your disability application.
