Standard Form (SF) 3112D is entitled “Agency Certification of Reassignment and Accommodation Efforts” and is contained in SF 3112: “Documentation in Support of Disability Retirement Application.” Note that in order for you to be able to prove that you are eligible for FERS disability retirement benefits, your agency must verify that they are unable to accommodate your medical condition and that they are unable to reassign you to a vacant position for which you are minimally qualified and for which you are able to perform all of the essential functions with or without accommodation. Your agency is encouraged to attach supporting documentation including medical reports and a detailed statement, which describes the requirements of the position and your inability to meet said requirements with or without accommodation. Be sure to review this form for accuracy and completeness before submitting to OPM as part of your application package.
