Standard Form (SF) 3112B is entitled “Supervisor’s Statement” and is contained in SF 3112: “Documentation in Support of Disability Retirement Application.” You must provide your supervisor a completed, signed copy of SF 3112A: “Applicant’s Statement of Disability” and ask your supervisor to complete SF 3112B. Note that in order for you to be able to prove that you are eligible for FERS disability retirement benefits, your supervisor must be able to verify that your performance, attendance, conduct, or a combination of all three is unacceptable and does not meet the standards set forth in your official position description. Your supervisor is encouraged to attach supporting documentation such as recent appraisals, time and attendance records, accommodation efforts, etc. Be sure to review the form for accuracy and completeness. Although your supervisor has the final say as to the content of SF 3112B, you should feel comfortable approaching your supervisor with any questions and/or comments you may have. For example, sometimes supervisors will include ambiguous statements that require clarification. Do your best to persuade your supervisor to provide statements that are clear and unambiguous, because you definitely don’t want OPM to reject your application because of a simple misinterpretation that could have easily been prevented.
