Standard Form (SF) 3107-1 is entitled “Certified Summary of Federal Service: Federal Employees Retirement System.” This form must be completed by your Agency on your behalf and included with your application for FERS disability retirement. All federal employees who are seeking retirement benefits regardless of the type (e.g., disability, MRA + 10, discontinued service, etc.) must submit a completed SF 3107 with their application package. Once your Agency completes this form, they should return the original to you for your review and signature. Please take the time to review this form carefully and question your Agency if you don’t agree with the listed service dates, etc. And please don’t forget to sign and date Section E, or you may experience delays in the processing of your disability application.
